All Swim4Elise Foundation scholarships require completion of a reference form in order to qualify for consideration.

  1. Based on the type of scholarship you’re applying for, select and complete the appropriate form on this page (including any required signatures), and save as a digital file.
  2. Upload the completed file where requested on your application.
  3. Submit your application!
